Though, some panels dont have the shirt designs on purpose..because they dont match the scene...
but in the first panel here, the shirt designs are supposed to be there.

yes, scarring is no big deal. One time I coughed up blood too...no biggie. Your throat gets scratched and scarred when you cough a lot...so...*shrug* it's not like it's straight from the lungs. XD
